Special Notice for BCBS MMAI Patients

The Illinois Eye Institute is a nonprofit eye center that provides comprehensive eye care to individuals of all ages in the Chicagoland area. We strive to meet all patients’ ocular and vision needs, regardless of insurance or financial status.

This is a special notice to our BCBS MMAI patients:

​

Blue Cross Blue Shield (BCBS) MMAI (Medicare-Medicaid Alignment Initiative) will end in Illinois on December 31, 2025, and members will be switched to a new Illinois FIDE SNP (Fully Integrated Dual Eligible Special Needs Plan).

​

This is part of a transition where MMAI plans are being replaced by FIDE SNP plans for people in Illinois with both Medicare and Medicaid.

 

BCBS has notified members of this change and you will need to choose a new plan, though current members can stay with their existing MMAI plan until the end of the year.

If you want to continue care with the Illinois Eye Institute, we currently contracted with:

​

 

  • Humana MMAI (Humana Dual Fully Integrated)

  • Meridian Dual Fully Integrated

What this means for you

Coverage ending:

Your BCBS MMAI coverage will end on December 31, 2025. 


Switch to FIDE SNP:

You will be automatically moved into a new Illinois FIDE SNP plan starting January 1, 2026. 


Keep your plan until 2026:

You can stay in your current MMAI plan until December 31, 2025. 


Notification:

You should receive letters from BCBS about the transition and the need to choose a new plan

The Illinois Eye Institute is contracted with the following:

​

  • Humana MMAI (Humana Dual Fully Integrated)

  • Meridian Dual Fully Integrated

​

The Illinois Eye Institute is not contracted with:

  • Aetna Better Health MMAI

  • Molina MMAI


Check provider network:

It is important to check if your current providers are in the network for the new FIDE SNP plan, as networks can change. 


How to get help:

Call Member Services at the number on your member ID card or visit the BCBS website for updated provider information, as you will need to choose a new plan for 2026.
